Heat stroke is a severe medical emergency that occurs when the body's temperature regulation system collapses due to prolonged exposure to extreme heat. Especially when temperatures soar above 40°C during our hot Turkish summers, the body may fail to cool itself sufficiently through sweating, putting internal organs at risk of severe damage. Once the brain's temperature control center is disabled, body temperature rapidly reaches critical and dangerous levels.

What is Heat Stroke (Sunstroke)?

The center responsible for maintaining heat balance in the human body is a region in the brain called the hypothalamus. Under normal conditions, when you are in a hot environment, the blood vessels on the surface of your skin dilate, blood flow increases, and sweating begins. As sweat evaporates from the skin, the body cools down. However, in extremely high temperatures, especially when humidity is high, this mechanism falls short. When the hypothalamus loses its function, the body cannot shed enough heat to return to its normal temperature. Body heat rises rapidly, beginning to damage cells and tissues. While heat stroke is typically seen in weather above 40 degrees, it can occur at lower temperatures in vulnerable groups such as children, the elderly, and those with chronic illnesses.

Differences Between Heat Stroke and Heat Exhaustion

The initial stage of a heat stroke is usually referred to as heat exhaustion. During heat exhaustion, the body has lost excessive fluids and salt, but the temperature regulation mechanism is still struggling to work. At this stage, the person sweats profusely, their skin is pale and clammy, and they feel intense thirst. If heat exhaustion is not taken seriously and the person does not move to a cool place and hydrate, the condition progresses to a heat stroke. In a heat stroke, sweating stops completely. The skin becomes red, hot, and dry. While heat exhaustion can be managed with rest and hydration at home, a heat stroke is a potentially fatal emergency that requires direct hospital treatment.

Causes and Risk Factors

The fundamental cause of a heat stroke is the body being exposed to more heat than it can tolerate or generating excessive internal heat. Working unprotected under the sun for long hours during the summer, sunbathing for hours by the sea along the beautiful coasts of Türkiye, or engaging in heavy sports during the hottest parts of the day are the most common triggers. High humidity makes it difficult for the body to cool down because it prevents sweat from evaporating, thereby increasing the risk. Wearing thick, non-breathable clothing, overeating, or consuming alcohol are also factors that raise body temperature.

Anyone exposed to the sun for extended periods can face this danger, but certain groups are at a much higher risk. The temperature regulation systems of babies and young children are not yet fully developed. In elderly individuals, this system weakens over time. Those with chronic diseases such as diabetes, hypertension, or arrhythmias, and those taking diuretics or blood pressure medications are more sensitive to the heat. Additionally, individuals with congenital sweating disorders, marathon runners, and those with neurological conditions like Alzheimer's are also in the high-risk group.

What Are the Symptoms of Heat Stroke?

Heat stroke symptoms escalate rapidly as the body temperature climbs above 40°C. The most striking finding is neurological changes. The person may begin to show mental symptoms such as confusion, disorientation, irritability, slurred speech, or hallucinations. In advanced cases, seizures and complete loss of consciousness (coma) can occur.

Among physical symptoms, the cessation of sweating stands out; the skin is extremely hot to the touch, dry, and usually flushed red. Because the heart is trying to pump more blood to cool the body, the pulse beats very rapidly. This is accompanied by fast and shallow breathing. A severe throbbing headache, nausea, vomiting, dilated pupils, and severe muscle cramps are also common heat stroke symptoms. When any of these symptoms are noticed, even seconds are of great importance.

Diagnosis and Diagnostic Methods

When a patient is brought to the hospital, a heat stroke diagnosis is generally made through a physical examination and an assessment of the environmental conditions the patient was exposed to. The most accurate measurement of core body temperature is taken rectally, as ear or armpit measurements may not reflect the true temperature of the internal organs. A body temperature of 40°C and above confirms the diagnosis.

In addition, various tests are requested to determine if there is any damage to the internal organs. Blood tests check electrolyte levels such as sodium and potassium and assess kidney function. Urine tests are used to examine dark-colored urine, which is an indicator of muscle breakdown (rhabdomyolysis). Furthermore, additional laboratory analyses are performed to detect damage in muscle tissue and evaluate blood clotting status.

Heat Stroke Treatment and First Aid

Treating a heat stroke is a race against time. The initial intervention must be done at the scene, while the definitive treatment must take place in a fully equipped hospital.

First Aid for Heat Stroke

If someone around you is experiencing a heat stroke, you should immediately call the 112 emergency hotline in Türkiye. Until the ambulance arrives, move the patient to a shaded and cool area right away. Remove any excess and tight clothing. Cooling the patient rapidly must be your top priority. If possible, immerse the patient in a cold water bath or spray them with a hose. If this is not possible, wrap the patient's body in wet cloths. Apply cold compresses or ice packs specifically to the neck, armpits, and groin areas; large blood vessels are very close to the skin in these regions, which helps cool the blood. Slightly elevate the patient's feet so blood can reach the brain more easily. If they are unconscious, absolutely do not try to make them drink water, as you could cause them to choke.

Hospital Treatment for Heat Stroke

In the hospital, emergency room teams apply aggressive cooling techniques to safely lower the body temperature to 38.5°C. Special cooling blankets or ice water applications may be used. Simultaneously, the patient is given intravenous fluids and electrolytes (sodium, chloride).

Important Warning: In the event of a heat stroke, never give the patient fever-reducing medications like paracetamol or aspirin. These drugs are effective for fevers caused by infections; they have absolutely no benefit for body temperature increases due to environmental heat. On the contrary, they can cause permanent and fatal damage to the liver and kidneys, which are already heavily strained by the extreme heat.

During the cooling process, the patient may start to shiver. Because shivering causes the body to generate heat, doctors may administer muscle relaxants to stop it. The patient may require monitoring in intensive care conditions until their situation stabilizes.

Ways to Prevent Heat Stroke

Preventing heat stroke is much easier and more effective than treating it. Maintaining your body's fluid balance during the summer months is the most crucial step. Drink water regularly without waiting to feel thirsty. Make sure to consume at least 2-2.5 liters of fluid a day. The color of your urine is the best indicator of adequate hydration; your urine should be pale yellow or clear. A dark yellow color indicates that the body is dehydrated. To replace the salt lost through sweat, you can opt for beverages like slightly salty ayran (a traditional Turkish yogurt drink) in moderation.

Try not to go outside between 10:00 and 16:00, when the sun's rays are the most direct. If you must go out, wear light-colored, loose-fitting clothes made of breathable fabrics like cotton or linen. Use a wide-brimmed hat and sunglasses. Postpone heavy physical activities to the early hours of the morning or the cool of the evening. Complications and Risk Groups

If a heat stroke is not treated correctly and swiftly, it can be fatal. If the treatment is unsuccessful, the patient can be lost within half an hour. Even in some patients who survive a severe attack, organ loss or death can be observed days later due to complications such as brain edema, acute kidney failure, liver damage, or heart failure.

Heart patients and individuals with hypertension are at particularly high risk during this process. Extreme heat increases the heart's workload and can trigger a heart attack. When to See a Doctor (Emergencies)

Any situation with a suspected heat stroke is a medical emergency, and 112 should be called without losing time, or you should head to the nearest emergency room. Seconds are especially precious if the following symptoms are present:

  • The person's consciousness becomes cloudy, they give nonsensical answers to questions, or they faint,
  • The skin becomes completely dry, hot, and red,
  • Inability to take fluids orally due to severe vomiting,
  • Experiencing shortness of breath, chest pain, or seizures,
  • The body temperature does not drop within 30 minutes despite applied first aid and cooling procedures.
